The Provider Registration Packet consists of three forms: (1) The Provider Registration Form, (2) The Provider Participation Agreement, and (3) The W-9 Form.

Please remember this key information when registering Health Aides:

Only the original AHCCCS forms and photocopies of the originals are accepted.  Re-typed or scanned forms will NOT be accepted.

ORIGINAL SIGNATURES ARE REQUIRED IN THE FOLLOWING SECTIONS:

Line 48 of the Provider Registration Form

The Provider Participation Agreement Signature

SIGNATURE COPIES WILL BE ACCEPTED ONLY IN THE FOLLOWING SECTIONS:

Section IV, line 36 of the Provider Registration Form

The W-9 Form Signature

Stamped signatures are never allowed.

The registration process for Health Aides must mirror the process for any other AHCCCS provider.  Please follow the above requirements when registering Health Aides.  If these requirements are not met, AHCCCS will not be able to process the registration packet and it will be returned to the LEA for corrections.

We appreciate your efforts in this matter, and if we can be of assistance in anyway please feel free to contact us at 480-736-0195.
