DSC Questions

1. How do I obtain service logs for my districts providers to fill out?

Your MAXIMUS Account Manager can provide you with service logs. If your LEA contracts with a billing agent, they also can provide you with the service logs needed for completion.

2. How do I train the providers in my district to fill out a service log?

Your MAXIMUS Account Manager can provider training, free of charge, for your providers, or you can attend one of our regional "Train-the-Trainer" sessions that is offered several times throughout the year. Please contact your Account Manager for more information.

3. Do I have to use a billing agent?

Although using a billing agent is not a program requirement, MAXIMUS recommends you speak with all billers to see if they can fit the needs your district requires as DSC billing can be complicated.

4. How do I register a new provider with AHCCCS?

AHCCCS requires that paperwork is completed for all providers. MAXIMUS is happy to assist LEAs in completing the required forms. For more information please contact your assigned MAXIMUS Account Manager.

The forms required to fill out are as follows:

· Provider Registration Form
· Provider Participation Agreement
· W-9

In addition to filling out these forms, submit a copy of the provider's license. Once completed mail to:

5. How do I link a provider to my district if they already have a provider number with AHCCCS?

You will need to fill out the following form(/s):

MAC Questions

1. If my LEA signs a Medicaid Administrative Claiming (MAC) Participation Agreement with MAXIMUS does it mean mandatory participation?

Signing an agreement does not bind you to participation in the MAC program. It is simply an agreement between the LEA and MAXIMUS that outlines the responsibilities of each party.

2. How do I know who to include on my personnel roster?

LEAs should include employees and contractors that are not 100% federally funded and who ROUTINELY perform Medicaid Administrative and Outreach Activities.

3. How much of an impact will participation in the time study process have on sampled district staff?

Each RMS (random moment sample) Observation form takes approximately one minute to complete. In general, the paperwork associated with participation in the MAC program for sampled personnel will be less then 20 minutes over an entire school year period.

4. What are common reasons RMS Observation Forms are returned to LEAs?

MAXIMUS will return RMS Observation Forms to LEAs if they are not filled out correctly. Common reasons include:
· The form was filled out before the sampled moment in time;
· The participant didn't sign / date the form appropriately;
· The form wasn't validated if needed;
· The form has white out (other corrective tape / liquid) on it;
· The answers to the three questions do not clearly explain what the individual was doing at the sampled moment ; and/or
· The participant didn't select a job code


5. How often should we send in our RMS Observation forms?

After making a copy of the completed forms, please mail all collected forms on a weekly basis to:

6. If an employee has lost their RMS form, can it be recreated?

MAXIMUS encourages MAC Coordinators to make copies of the RMS Observation Forms upon receipt from MAXIMUS. However, lost forms can easily be recreated by using the sample RMS Observation Form that is available on the download page (left side of the web-site).

7. What influences the reimbursement amount LEAs receive in the MAC Program?
The reimbursement amount for the MAC Claim is based upon four variables:
· Percentage of time identified as reimbursable (statewide - based on results of RMS process);
· Costs (LEA specific - costs for individuals on personnel roster and allowed district wide expenditures);
· MER (LEA specific - percentage of LEA students eligible for Medicaid Title XIX services); and
· Non-restricted Indirect Cost Rate (LEA specific - rate set by Arizona Department of Education)
