To begin participation in the program, a LEA has to simply call the MAXIMUS office and we will provide you with a participation agreement.  All participating LEAs are required to sign a participation agreement (contract) with MAXIMUS.  The agreement details the responsibilities and rights of both MAXIMUS and the participating LEA and the services to be provided to the LEA.  Each program, MAC & DSC, requires its own contract to be completed. The completed contract allows the LEA to participate in the school based Medicaid program administered by MAXIMUS. Upon the completion of this contract, MAXIMUS will work with the LEA to implement the Medicaid claiming process.
